This template is provided for general informational purposes and is not legal advice. Consent requirements vary by airline and destination country, and notarization is often recommended — verify the rules for your specific trip and consult a qualified attorney with questions about custody-related travel.
Why a travel consent letter matters
When a minor travels internationally with one parent, a grandparent, another relative, or a school group, border officials and airlines may ask for proof that the absent parent or parents consented to the trip. A child travel consent form is that proof: a signed letter from the non-traveling parent(s) authorizing the journey. U.S. Customs and Border Protection recommends carrying one, and some countries — Canada and Mexico among the strict ones — routinely ask for it at entry.
What the letter should contain
Include the child's full name, date of birth, and passport number; the names of the adults the child is traveling with; trip details (destinations, dates, airline if known); contact information for the consenting parent(s); and an emergency medical authorization line so the accompanying adult can approve treatment if needed. The consenting parent signs and dates the letter — and although not always legally required, having the signature notarized is widely recommended because border agents give a notarized letter far more weight.
Preparing it online
Fill in every detail in the FinishMyDocs editor — much cleaner than a handwritten note drafted at the airport — then download and print the letter for signing. If you're notarizing, sign in front of the notary, not before. Make two printed copies: one for the traveling adult's carry-on and one left at home with the itinerary.
Requirements vary by airline and destination country, so check the embassy or airline guidance for your specific trip a few weeks before departure.
